Legal Name
Please note that, by default, the legal name you provide will be listed on a staff
directory, and that reasonable measures have been taken to ensure the staff directory will only
be accessible to EFNW staff, partners, agents, and other authorized personnel. To request that
your legal name not be included in that staff directory, please email hr@everfreenw.com with your request. Legal names will
still be retained for reasonable business purposes, and will be released if required by law.
Preferred Name
Since some people prefer to go by their “Fan Name” and others prefer their real
name, let us know what name you prefer to go by. This is also the name that will be printed in
the staff section of the con book.
Preferred Username
Everfree Northwest has lots of tools available to our staff, such as a staff forum.
Let us know what username you'd like to have.
(Usernames can have letters, numbers, hyphens (-), and underscores (_), but no spaces.)
Other Aliases
In case you have any other handles people might know you by, let us know so that our
staff will recognize you!
Discord ID
A Discord ID is not required to apply, but most Everfree Northwest communication will take place through Discord.

Birth Date (MM/DD/YYYY)
To volunteer at Everfree Northwest 2018 you must be 16 years of age on or before
May 17, 2018. To volunteer without a parent or guardian's approval you must be 18 years of
age on or before May 17, 2018. If you are not going to be 18 years of age by this date, a
Human Resources representative will supply you with a "Minor Volunteer Agreement."

Even though Everfree Northwest is only 3 days out of the year, months of physical
preparation goes into our event! Although it's not required in order to volunteer, we're looking
for every helping hoof to help move, carry, inventory, setup, and create our supplies,
regardless of your department.

Street Team is an opportunity for staffers and attendees alike to promote Everfree
Northwest in their local communities in exchange for perks. If you select “Yes” you will be
contacted at a later date with more information.

Do you intend to Cosplay during your assigned shifts as a staff member?
Cosplay is allowed and encouraged for most positions on staff. However, some
positions require a more professional appearance, and therefore may not be allowed to cosplay
while on duty. Be sure to contact your department's director and make sure that cosplay is
appropriate for your position.
